I have decided to take a break from current clinical practice and explore other interests – and will hopefully return to clinical practice again. As I prepare for this next chapter, my primary concern is to ensure that you continue to receive the highest standard of specialized care. Jiva Health:
To ensure your health needs are met without interruption, I have selected a healthcare company called JivaHealth (https://jivahealth.com) and am pleased to announce that Jiva Health will be taking over my practice and believe that they are capable in providing the specialized Pulmonary and Sleep medicine services you require and are accustomed to in my office over the years. Jiva Health will maintain your treatment plans without any disruption. Please call the team to confirm your appointment.
You are not bound to follow with team Jivahealth as a successor of my practice and can always choose another pulmonary and / or sleep physician of your choice from the community, if you so desire. Please ask Jiva Health for your records to be transferred per their protocols to the new physician.
Continuity of Care:
– Your medical records will be available to the Jiva Health team and secure in EPIC. You do not need to take any action to transfer your files. You can always request your files and records from the team at Jiva Health, moving forwards.
– Office phone number (408) 942-0300, fax number (408) 890-7298 and the location: 995 Montague Expy, Suite 218, Milpitas CA 95035 will be maintained by Jivahealth moving forwards. You don’t have to remember new information.
– Any appointments scheduled on or after February 5th will be handled by Jiva Health. Please confirm with the team by calling the same number as before.
– Jiva Health will maintain same pulmonary and sleep medicine standards that we have always prioritized and concurred.
